For your convenience we have made available below all of our new patient forms.  These forms are available as a PDF file and should be filled out online, then either downloaded and FAX’d to our office (see contact info on this website for FAX numbers) or sent electronically via the internet (directions at the end of the forms).  All forms sent over the web are automatically encrypted and password protected for security purposes and can only be opened by our office staff from our secure site.

We appreciate you taking the time to fill out the forms ahead of your visit to our office.  This will enable us to spend more time discussing your orthodontic concerns at your appointment.
